What are the differences among "So", "Too" and "Very"? When and where should I use one instead of the others?

This answer is very subjective since it's our conclusion after doing some research around. Feel free to add comments so that we can improve.

So: normally appears with that

Very: extremely but manageable: She is very heavy. => you can use this in your IELTS Writing or for formal situations.

Too: excessively extreme and unmanageable: She is too heavy to ride a scooter => The same as "So". It should not be used in formal English.

For Example:

Your writing is very good!

His essay was so bad that he failed the subject.

She is so beautiful. => this use is considered casual English, it is useful in conversation, but you should avoid using it in your IELTS Writing Tasks.
