Familiar with = well informed about or knowing thoroughly. For example:
I'm familiar with his art work.
Are you familiar with the problems that usually arise in this shop?

Familiar to = recognizable. For example:
Your friend doesn't look familiar to me.
His art work is familiar to millions of people.
